Warranty Registration

To make a claim to EasyLift™ under the terms of this limited warranty, the original purchaser needs to provide this information in writing addressed to:

1. Full name and address of person making claim (place of installment)
2. Model of cargo lift
3. Date warranty was registered
4. Date of purchase and seller
5. A specifically defined description of the suspected defect
6. Name, address and telephone number of the installer
7. Return authorization number from EasyLift is required before any claim can be addressed
